If you're running out of medications, first check with your pharmacy to ensure you don't have refills. If you are only missing a few doses before the next appointment, the pharmacist can usually dispense a small emergency amount.
If you have no appointment booked and you're running out of medications then you are likely due to book an appointment to review medications in more detail.

For simple requests you can use the form below.
Examples of uses for the form:
- If the pharmacy didn't receive the prescription and it needs to be re-faxed
-If we adjusted the dose and it is working well, now you need more of the new dose
-If we adjusted a dose and it didn't work well and you want the previous dose instead
-If you need the same dose that is working well and our next appointment is a few weeks away or the pharmacist is unable to give you an emergency supply.
