Dear educators & teachers, thank-you in advance for the time it takes you to complete these forms and provide feedback on your students. Understanding how a child or youth does at school is a very important part to understanding their development so that I can recommend supports & resources.
The information collected on these forms is through a secure platform. Please ensure you include the student's name so it can be linked to their chart. The information will be incorporated into their medical record.

*Once opened the questionnaire select either :
1) Intake - new assessments: these are forms if the child/youth is meeting the pediatrician for an assessment
Then to make the questions appear - select if you are a teacher at a daycare, elementary school or middle/high school (different questions for different age groups)
2) Symptom check-in: these are forms teachers to complete a pre appointment check-in for youth seeing the pediatrician for ADHD medications or follow up to rate current ADHD symptoms for a child or youth.
